- Two (2) Photocopies of PSA Birth Certificate

- Transcript of Records for evaluation
- Honorable Dismissal
- Two (2) photocopies of PSA Birth Certificate

- 96 -100 GWA – for incoming freshmen
- Tertiary Education Subsidy
(Php 60,000 per beneficiary)
